WEST PLAINS, MO – Ozarks Healthcare has been selected as a Hub Anchor organization for Missouri’s Rural Health Transformation Program (RHTP), a statewide initiative designed to strengthen rural healthcare through collaboration, innovation, and community-driven solutions.
The Missouri Department of Social Services (DSS) selected Ozarks Healthcare as one of 27 Hub Anchor organizations across the state to help lead implementation of the Transformation of Rural Community Health (ToRCH) Care model within designated rural community hub areas.
The selection follows a competitive review process that evaluated organizations based on readiness, governance capacity, community partnerships, staffing plans, operational capabilities, and alignment with the goals of Missouri’s Rural Health Transformation Program.
As a Hub Anchor organization, Ozarks Healthcare will help bring together healthcare providers, community organizations, and local leaders to improve coordination of care, connect individuals with resources, and support innovative approaches to addressing the unique health needs of rural communities.

Through the ToRCH Care model, participating organizations will work collaboratively to create a more connected healthcare experience for rural residents by addressing healthcare needs both inside and outside traditional clinical settings.
